Post Your Face: Wide Nose, Worry Lines & Heavy Eye Lids

In our weekly Post Your Face column, Dr. Robert Freund answers readers' cosmetic surgery questions, and shows them how they'd look if they underwent the enhancement. To post your own face, visit postyourface.com.

Chrissy's Question: My nose is very wide, I wish it was smaller. I have a deep worry line in between my eyes, and my eyelids have heavy flaps.

Before (left) and after (right) alar base reduction to the nose and Dysport/Botox to the forehead. Courtesy Photo

Dr. Freund's Answer: Thank you for your pictures. You describe three concerns; 1) the width of your nose, 2) the worry lines between your eyebrows, and 3) your heavy upper eyelids. I will address the nose first and then the second and third concerns together.

The width of your nose: A simple rule of thumb is that a vertical line drawn from the inner corner of your eye should fall somewhere near the outer edge of the nostril. This can be seen in the photos by the blue lines. In the "Before" photo, the width of the nostrils is much greater than this line. In the "After" photo, these lines are much closer. One note of caution -- anyone rushing to a mirror to determine if their nostrils are too wide should evaluate the width without smiling. As is the case in your photos, smiling will falsely make the nostrils appear wider than they are. Plastic surgeons perform an alar base reduction or ABR, to narrow the width and opening size of the nostrils. By itself, ABR will cost from $2500 to $5000 and can be performed under local anesthesia. Downtime is 2-3 days with the sutures removed on the 5th day.

Your worry lines and heavy upper eyelids: It appears that the heavy upper lids are not so much the result of excess skin but more the result of some forehead and eyebrow sagging, more on the right than on the left. The worry lines are the result of too much frowning or excess facial expression. We could certainly lift the forehead surgically (a.k.a. Browlift), but can also lift the brows and eliminate the worry lines by using a muscle paralyzing agent (Dysport or Botox). These agents will weaken the muscles that make the lines and can also be used elevate the eyebrows. This unique combination of relaxation and lifting requires that the doctor is well-versed on facial anatomy and is experienced with the use of these agents. You can see what this treatment can do for you in the after photographs. These agents will cost around $750/treatment and will need to be performed 2-4 times per year. Downtime is minimal. Many people can return to work on the same day.

Respectfully submitted,

Robert Freund, MD, FACS

Great response! We all should be happy with what we have. No one is perfect. And sometimes it's the little imperfections that make a person desirable. Surgery should be an option for those with severe -quality of life problems.

Her nose is fine. Her face is fine. She looks sweet and NORMAL. Not everyone needs to look like a barbie. There's an obsession with perfection here that I simply do not understand.

Her nose gave her character, I don't think it looked wide at all. Now, it just looks kind of fake. It is not the right nose for her face structure.

She looks good, i guess we all would like to change something about ourselves, we as in women but if we sit back and let mother nature do her thing we'll see that we were beautiful all along. Remember beauty is only in the eye of the beholder!!

It's sad that we have so many real problems facing Americans and the world in general today. What's most sad, though, is that people are more worried about how they LOOK than what is in their CHARACTER. Maybe that's why the world is in the shape it's in. I don't know Chrissy, but she's got a great smile and what seems to be an honest face. Ditch the plastic surgeon and make the world a better place, Chrissy.

She doesn't need a nose job; she just needs a little make-up and an updated hairstyle. That will draw the emphasis away from her nose which isn't all that bad to begin with. It wouldn't hurt to speak to a self-esteem coach, either.

Chrissy,

Don't alter your nose. I think you are cuter with the wider pug nose. You are more of an individual. Altering your nose to make it a little thinner will not change your life. Invest the money in education or retirement, and you will reap a much greater reward. Those who really love you will love you regardless, and your face will change as you age. You will gain weight and your face will grow into a more symmetrical proportion.

I am not a doctor, only a woman who thought my nose was too wide when younger. Now that I have gained weight, my nose looks tiny in photos.

Think about it.

The smaller nose looks Michael Jacksonish. Oh yeah lets tell her the rule of thumb for making plastic ugly people so she thinks her nose is gimongous just so you get more business. You suck Dr! Keep your nose girly or when you have grandchildren you wont see a resemblance besides its CUTE! Ive seen wide noses in my time and yours aint one of them.

Me Never. I will never cut my face to look better than before. I am satisfied as I look. I can however, use chemicals to remove unnatural blemishes caused by environment such as cold, heat etc. God gave me my face and I should die with it. My kids look like me and I love it. If I change my looks I will notlonger look like my kids. People willl think they are not my kids. I have even told my kids never to have tatoos-permanent blemishes on the body.

With all due respect to the Doctor who stands to make a lot of money out of promoting alterations, your face is fine. Save your money I'm with Amy; get a nice haircut.

She definitely would benefit from a more flattering hairstyle. Botox would be a good start. The surgery on the other hand would been extreme. Only if her job requires her to run with the barbi and kens, otherwise some counseling might be in order first. The nose and ears continue to grow over our life times and the surgery changes as we age along with gravity changes. So go to counseling and volunteer at a senior center and come to grips with your aging process.

Chrissy,
You are unique the way you are, and these changes will not enhance your uniqueness. Surgery is ALWAYS a risk. Look at Priscilla Presley. I just had back surgery to correct my limp and pain from foot drop. I now cannot walk without the aid of a walker/cane because I have weakness in both feet now. But the good thing is my pain is gone. Surgery is not something to be done for vanity. I wonder if Kanye West's mother would still have had her cosmetic surgery if she knew it would cost her her life. What makes a woman beautiful has little to do with what is on the outside, and everything to do with that inner glow. Phsical beauty changes with time. Inner beauty does not as long as the joy you have from being uniquely you shines through. There are many more problems in this world to worry about. Please don't take an unnecessary rish.

I just can't believe people are so damn vain, why do people think this way? you are who you are, your a human being its so so stupid to do these things to yourself, and theres alot of things to worry about in life and your looks are not one of them, it seems people who do this are spoiled little brats whos way of thinking is so very screwed up, leave your body the way god made it and use the money for important things, some people don't have a dime and your spending thousands for stupid things like this!!!
