Post Your Face: Wide Nose, Worry Lines & Heavy Eye Lids

In our weekly Post Your Face column, Dr. Robert Freund answers readers' cosmetic surgery questions, and shows them how they'd look if they underwent the enhancement. To post your own face, visit postyourface.com.

Chrissy's Question: My nose is very wide, I wish it was smaller. I have a deep worry line in between my eyes, and my eyelids have heavy flaps.

Before (left) and after (right) alar base reduction to the nose and Dysport/Botox to the forehead. Courtesy Photo

Dr. Freund's Answer: Thank you for your pictures. You describe three concerns; 1) the width of your nose, 2) the worry lines between your eyebrows, and 3) your heavy upper eyelids. I will address the nose first and then the second and third concerns together.

The width of your nose: A simple rule of thumb is that a vertical line drawn from the inner corner of your eye should fall somewhere near the outer edge of the nostril. This can be seen in the photos by the blue lines. In the "Before" photo, the width of the nostrils is much greater than this line. In the "After" photo, these lines are much closer. One note of caution -- anyone rushing to a mirror to determine if their nostrils are too wide should evaluate the width without smiling. As is the case in your photos, smiling will falsely make the nostrils appear wider than they are. Plastic surgeons perform an alar base reduction or ABR, to narrow the width and opening size of the nostrils. By itself, ABR will cost from $2500 to $5000 and can be performed under local anesthesia. Downtime is 2-3 days with the sutures removed on the 5th day.

Your worry lines and heavy upper eyelids: It appears that the heavy upper lids are not so much the result of excess skin but more the result of some forehead and eyebrow sagging, more on the right than on the left. The worry lines are the result of too much frowning or excess facial expression. We could certainly lift the forehead surgically (a.k.a. Browlift), but can also lift the brows and eliminate the worry lines by using a muscle paralyzing agent (Dysport or Botox). These agents will weaken the muscles that make the lines and can also be used elevate the eyebrows. This unique combination of relaxation and lifting requires that the doctor is well-versed on facial anatomy and is experienced with the use of these agents. You can see what this treatment can do for you in the after photographs. These agents will cost around $750/treatment and will need to be performed 2-4 times per year. Downtime is minimal. Many people can return to work on the same day.

Respectfully submitted,

Robert Freund, MD, FACS

As a society,we need to stop this obsession with "perfection." We are all individuals. We don't all have the same jobs or the same houses, why should we all have the same faces? It's ridiculous.

Who makes up these "shoulds" and "rules of thumb"? People who stand to make gobs of money off plastic surgery, cosmetics and today's fashion must that becomes tomorrow's passé, that's who. Women, please don't listen to them! Especially when men aren't expected to change how they look in order to be considered socially acceptable...

Dear Chrissy,
Tho some of these comments r a little harsh, these ppl r right. You look better with the nose u were born with. I kno sometimes we feel something els will look better than what we hav now, but lik Debbi said in time our body and looks change. I think u r beautiful as u r...the updates only seem to mak u look younger; which is not always best. Ne way I just ask that u look at u nd try to lik wat u see. U r beautiful nd remember not all procedures turn out the way u invisioned them.

I also think she's fine and absolutely lovely just the way she is. But if she wants a little enhancement here and there, why criticize her for it? We all do stuff to our faces and hair to look nice. If she wants to take it to a surgical level, and if she wants to pay for it, and if she gets somebody who knows what the hell he/she is doing so it doesn't look like a "job," why the hell not? I'd rather do all my enhancements with foundation and hairspray, but Chrissy you do what makes you feel good. Get somebody good and pay what the procedures are worth....it's your face!

I' m sorry I'm going to disagree with the doc. While your nose is fine like all the guys say your nose is longer than the nose length of other girls and the tip of your nostrils are a little wider. May be you have been picking your nose. I mean all the girls are having nose jobs and you see button nose and compare your nose to theirs. They drive their parents to poor houses for Rhinoplasty jobs. Do not do that to your parents. You are a pretty girl. If you have big boobs forget about your nose. Guys with flat girls make them very conscious by staring at the boobs of girls with big boobs. So if you are going to think of your looks think of your boobs..

Nothing wrong with Chrissy's face. She should spend the $2500-$5000 on a tropical vacation instead.

"Excessive facial expression" .... Yes, by all means, stop frowning, stop smiling, stop laughing so hard your face contorts, or crying in grief or joy. Good God people, beauty comes from the INSIDE.

hi all - thanks for "some" of your comments about how my nose isn't so bad! My husband would leave me if I got plastic surgery just FYI -- i DO think my nose is a bit wide, but I did this for fun. Who wouldn't want to see how a plastic surgeon might imagine them! If you've got the guts go to postmyface.com and try it for yourself! Now as for the comment that I look like Russell Crowe... that's a first! I've gotten Monica Seles and Celine Dion - but never the Gladiator. Maybe I do need plastic surgery!

I just wonder who made the "rule" that your outer nose line should be a straight line from the inner corner of the eye... or any beauty rule for that matter!!!!
Everyone is looking so plastic these days. She is a beautiful girl before the proceedure already.
While I think sometimes proceedures might be necessary for some people, I think most women are trying so hard to be beautiful on the outside, that they've become superficial, surface only human beings with no depth to their souls.
If people would just try a healthy balance of inner beauty and outer beauty, this world would be a much better place.

Chrissy, your nose is not too wide. You are very pretty, naturally. Other people have mentioned make-up and hair cut. That will bring out your natural beauty. I had rhinoplasty years ago to fix a defect. I wish I could have avoided that. You want to do something? Go to SmileTrain.org and OperationSmile.org. You will cry. You will thank God for what you have. Make a donation. Make a difference.

Your picture looks like you have a nice sweet personality. Don't alter it (remember the old saying: "Don't cut off your nose to spite your face." Seriously, don't do anything different. Check out this site, it will change your mind if your husband cant... www.awfulplasticsurgery.com/category/bad-nose-jobs.

Is there a site where botched breast reconstruction, ruptured implants causing both pain and deformaties, and women who have never had reconstruction after mastectomy? I also know a few people who has fat removed that has left "holes" in thier belly and thigh. People who have lost alot of weight resulting in large flaps of skin on thier arms, neck and large belly "apron" would be very interested in your opinion. All the people who say beauty is only on the inside, some things are inside and out. Counseling is important, many of which reccomendations are to become comfortable with the idea of reconstruction or other surgeries..

Sorry but I like the after picture and if she would feel more comfortable about her image because of the surgery, she should do it. As long as she's not changing her looks to please other people, but to feel better about herself, plastic surgery is okay.
She definitely could use a better hair cut, and I guess this is mean but what she could really use is a lip implant. Sorry but that's my opinion. =/

I think you look far better without the surgery and botox. All that those two things will do is give you an unnatural look. Your eyes right now have a sweet, soft look to them. Doing those things to them would make them strange, and the "botox" eyes don't go with the rest of your face. The same thing with her nose; it's nice now and it goes with the rest of her face.

For any girl thinking of getting plastic surgery, please don't. If you take care of yourself, it will bring out the unique things about your appearance naturally. For Chrissy, her eyes look really welcoming and sweet. I'd be willing to bet that if someone looks close enough, every girl has "something" about them. Not everyone may see it, but it's there. You don't need surgery to get that something, and in the process of getting plastic surgery you lose many things.

Who you are as a person, can make you more attractive visually as well as personally. It does give that "glow" that "Marnie" was speaking of in their post earlier. That is, if the person's not shallow, and they get to know you. The people that are NOT pervs, the people that take time for the right reasons to get to know you and what makes you special... they're the ones you should be around to begin with. Not the ones who want you to be fake, malleable.

Be you, and if you have faith look to God. Put great thought into whatever it is that you do or plan on doing, and think of the consequences. Something like surgery is a thing that can have many consequences, both obvious and hidden.

Take care.

For such little imperfection, I would not undergo plastic surgery. Get the nose inserts (you can get them at Amazon or NoseSecret.com). You will have a slimmer nose without the risk and pain of the surgery....super affordable with instat results.

And i cannot believe how insulted other people get when others change a thing or two about their selfs, dont tell me that what's inside is what really matters,bc/ that a LIE!!!!!,personality and looks matter, plus there is nothing wrong with being vain, after all it's her nose and she's free to do what she want with it,plus she looks younger and pretty.
