Post Your Face: 'My Jowls Are What's Bothering Me Most'

The Patient's Question: I need help with my eyes and my clown lines but my jowls are what's bothering me most.

Dr. Freund's Answer: Thank you for the submission. Your concerns are common among women in their late 40s, early 50s. Your upper eyes grow heavy and tired-looking from excess upper eyelid skin and sagging of the forehead. The lower lids appear tired-looking from the excess bags of fat under the eyes, as well as the hollowness below the bags that is the result of sagging of the cheek. When the cheek fat sags, it lands on the outside of the smile lines (nasolabial folds) and on the jawline as jowls.

The key to good, subtle eyelid rejuvenation is moderation. More than any other area of the face, the eyes can be permanently disfigured if too much tissue is removed. Removing too much upper eyelid skin can also make closing your eyes difficult; removing too much lower eyelid skin can give you that bloodhound look. My suggestion is for you to have an upper and lower eyelid blepharoplasty. Typically these procedures together range from $4,000 to $7,000.

As for your jowls, I'd recommend a vertical facelift to put the sagging cheek tissue back where it came from. You show a good deal of sagging tissue for a relatively young person, but vertically lifting the tissues will help to restore the cheeks, nasolabial folds, and jawline. The cost for this procedure is about $8,000.

The downtime for these procedures is 10 days to 3 weeks.

I am 49 years old and I have had my face lifted, brow lift, upper and lower eyes done and a tummy tuck. I don't mind getting old but I don't feel I have to look bad. I will have more plastic surgery as time goes on. it gives you a big boost for your self confidence.

Good for you. I wanted to have work done a couple years ago , but the market started going South---and so did all my retirement. If it ever picks up again the Plastic Surgeon's office is one of my first stops. :)

The first thing I noticed is her chin - wassup with that? It has an indentation but unlike a cleft chin which has a dimple, it's a long line. I would fix that first, it just looks so odd. She should also get a nose job at the same time too. It's really discerning to look at her face, because it looks like the whole thing is melting downwards, from her eyes to the end of her face, its all going south. She's really close to ugly, poor thing. Too bad they don't have that Swan show so she can just re-do her entire face. This pix just makes her look like an ex-con, all she's missing is an orange jumpsuit. Sorry but I call 'em as I see 'em.

This is a "hypothetical" picture..pretty misleading unless you look at her hair. They have photoshopped it to look different, but it's the exact same picture..just made to look different.

To Gladys: It is true it is very exspensive but I saved one year to have my face lift then I got financing for my tummy tuck. The interest rate is very low and they let you spread it out over an 18 month period.

Oh my goodness! I have the jowls also - but due to being unemployed now - I can even consider fixing them. I have always felt I looked good - even as I age - but now the jowls are overshadowing everything! I agree smiling helps - but you can't smile all the time or people will think that something is wrong with you! Is there some less expensive way to conquer the jowls?

I live in Chicago and took a plastic surgery vacation to Scottsdale, AZ. Everything turned out great and when I returned home, all my friends just thought I got some great rest and relaxation and did not make a big to do at my refreshed look. I had a mini facelift with some botox and skin treatment from a Board Certified
Scottsdale plastic surgery expert.

If any one wants to do the work. Using peanut oil every day and massaging the face as well as patting will do wonders to lift the sagging jowls. The patting is like an exercise for the face. I have done this for years & my face is doing quite well even though I have a familial tendency to have jowls & double chin.

Obviously you are a man. Probably a married one. Were you single, your last choice would be one of the ladies, growing old gracefully. Those of us who are at the age that we have the choice of surgery or growing old gracefully right now, are the ones that grew up being told that being beautiful and thin was the single most important thing. Girls went to college to meet a man. We bought books that told us how to greet our husbands at the door wrapped in saran wrap and a bow. Men generally dont want to get to know you unless you are appealing physically and that varies with each man, Until you know what it is like to be judged worthy of finding out if you are a good person by what your face and/or body looks like, you need to shut up.

I would consider having plastic surgery if I had the money. I am now 55 and showing my age. For years, I was always tolded I looked young for my age. I'm single now and I know men are looking at the younger ones. My own brothers, which are close to my age, don't want women their age either. It's hard to acknowledge that I'm not as attractive as I used to be. I don't get the looks like I used to from men anymore and that hurts.

To those who say "grow old gracefully"...and act as if getting a "freshening up" is somehow egotistical or foolish.... I would assume then that these people would never cap their teeth, or whiten them...and if they lose any teeth...just let the gaps show when they smile. That's what they used to do in the "olden, olden days" .. iti was nothing to see people who yellow and missing teeth. Would people accept that today? No.
Same way with "falling, sagging skin, causing the face to look TIRED and old. Why should you see someone else when you look in the mirror, if the technology today can improve the gravity forces and return your face closer to your "real" face?

I have not had any plastic surgery.... but would love to do so. One of the reasons I don't is choosing a doctor. There are many of them in the Philadelphia and suburb areas... but the doctor who does your work is a HUGE factor. Even the doctors with all the right credentials are not necessarily that good. That's why the cream of the crop is in NYCity, or California.... and maybe a few in this area. But you would probably have to rob a bank to pay one of the best plastic surgeons to do your work.

The other factor of course
