Post Your Face: 'I'm 69 and Concerned About Sagging Jowls'

In our weekly Post Your Face column, Dr. Robert Freund answers readers' cosmetic surgery questions, and shows them how they'd look if they underwent the enhancement. To post your own face, visit postyourface.com.

The Patient's Question: I tried Titan laser treatments to stimulate collagen and it helped but not enough. I'm 69 and concerned about sagging jowls, folds by the nose, low brows and eyelid folds. I also think I should have a small chin implant to balance the face.

Dr. Freund's Answer: First, please understand that in-office procedures like Titan laser treatments can only go so far. At your age, it's essential to have a daily regimen that cleanses without drying, and renews or protects the vital layers that provide strength and elasticity to your skin.

Here are 7 must-dos:

1. Wash daily with a non-foaming cleanser (Cetaphil)
2. Exfoliate twice a week with a Buf Puf or a mild facial scrub with salicylic acid.
3. Moisturize with any product that you like. I have no particular favorite.
4. Wear sunblock that contains zinc oxide (EltaMD or Blue Lizard)
5. Apply Retin-A (must be prescribed by your physician in order to be dosed properly) to thicken the skin, slow the loss of elasticity, even out the blotchiness and reduce your risk for certain types of skin cancer. Don't use it in the sun.
6. Apply anti-oxidants to help fight the production of free radicals (those bad chemicals that damage cells and make us age prematurely. Revale, Vitamin C (Cellex-C) or Prevage are all good.
7. Take Vitamin C or Fish oil and eat lots of colorful fruits and vegetables like grapes and strawberries that have loads of natural anti-oxidants.

As for in-office skin treatments...chemical peels or laser resurfacing will both tighten, firm and even your skin tone. Peels are less expensive ($150-300 per treatment versus $600-1,000 for a laser) and probably offer the same amount of improvement.

If you want surgery, the photos show how a facelift would lift the jowls and tighten the neck (A). An upper eyelift would also freshen your eyes (B). A chin implant will not make you look any younger but it will make you look different. If you've always disliked your chin, then an implant is a fine choice. It will cost about $3,000.

I've used some of the most expensive anti-aging and anti-wrinkle creams from the top lines in the world. I've had companies make "custom formulations" for my skin. I have to say that LifeCell ( http://tiny.cc/LifeCell )  beats them all. It is the ultimate anti-aging product. For those who want serious anti-aging skin care, this is the product to use, bar none.

My sister was considering laser resurfacing to get rid of her wrinkles and instead of going through that pain and expense, I urged her to reconsider and try Dermitage with me. For the age spots, use Retin A. It's expensive and a commitment, but it really works to eliminate age spots and wrinkles.

I would love if Americans, both women & men, would accept the beauty & inevitability of the aging process. I am an artist & there is nothing lovelier than a natural face, whatever the age. But those who think physical looks are more important than thrift & character will continue to waste money pursuing the fountain of youth... and showing how shallow you are in the process. My grandchildren marvel at my wrinkles & if they seem worried about them I assure them that "they don't hurt!" Accept the way life progresses. Be honest about your age & health. Take good care of your body & try to look your best but realize this is not all there is. Look ahead to the wonder of eternal life in heaven, where we won't grow old.

I worked for several cosmetics company in the past and also Ortho Pharmaceuticals, who created Retin A and Renova. Not too many things work well on the jowl line and would love that surgery procedure, for a cleaner and more youthful look.

I use Olay Regenerist Moisture lotion , the night cream. I would like a copy of the book to get more information to what and how to use on my skin and also for my 46 yr old daughter to start to use the proper products. I would like to have my eye lids lifted due to the fact they have become thin and sag. Also my under chin area is saggy and has some creases. Cheeks are getting a bit howled out in center.

I am 46, exercise regularly, and am experiencing the hollowing in the face as well. I was told by a very good friend to try Restylan or a "liquid face lift". WOW!...talk about taking off 10 years! I am beyond pleased with the results.
